Fire Risk Assessment

A Fire Risk Assessment is a comprehensive report completed by a qualified Fire Risk Assessor to identify any potential fire hazards, assess the risk associated with those hazards, and develop a list of actions to minimize or eliminate them. It involves checks on the building structures, fire safety systems, and evacuation procedures to ensure the safety of everyone within your building.

Mandatory Occurrence Reporting (MOR)

Announcement Important Update Regarding a MOR Occurrence

We are letting you know of a Mandatory Occurrence Reporting (MOR) Occurrence that was identified on 12/02/2026.

Type of safety occurrence:

Fire and smoke spread.

Describe the safety occurrence in detail:

The mechanical smoke extraction system is not working in the communal flat lobbies.

Tell us what caused or is causing the safety occurrence:

Colt have advised that the PSU is not providing 26v to the front door which has caused each damper to open on all floors.

Tell us who has been affected or who could be affected, and in what ways:

Primarily residents would be affected. If the AOV system is inoperative, smoke spreading into the lift lobbies and staircase would not be ventilated and could rapidly accumulate. Escape routes and stairwells could become unusable due to zero visibility and high toxicity, preventing residents from evacuating safely. Fire-fighting operations would also be hampered as firefighters rely on these systems to keep lobbies and corridors clear to access the fire. Inoperative vents may make it difficult for fire and rescue services to reach the fire-affected area.

Explain what you are doing or planning to do to keep people safe:

Colt have confirmed that they have ordered a replacement control panel and site attendance is booked for the 26.02.2026 to replace the panel and reactivate the system.

There are several manually openable windows in the staircase, and these have been left in the open position to provide some natural smoke ventilation if required.

Colt have confirmed that the system still operates to some degree, just not 100%, however, how the system would operate under fire conditions is not fully known.

The fire risk assessment has been reviewed, and this has not recommended the need for any interim measures at this stage.

Tell us how Residential Management Group discovered the safety occurrence:

The fault was reported to RMG by a contractor undertaking routine servicing of the AOV system.

Tell us anything you think should be shared for others to learn from:

Lessons have been shared internally regarding the reporting of faults to the FRS and MOR reporting to the regulator in-line with RMGs internal procedures however, it should be noted that the PM in this case was extremely diligent in reporting the issue to the relevant authorities.
